Obituary

Dr. Mark David  Siletchnik, 75,  passed away on November 7, 2024 after a long illness (Myotonic Dystrophy).  He was predeceased by his parents,  Leon and Edith Siletchnik, born and raised in Newport, RI.

He graduated in 1967 from Rogers High School, where he played the flute in the band and was active in ROTC. He graduated from the University of Rhode Island with a Pharmacy degree and from New York Medical College with a degree in Medicine and joined the U.S. Army. Mark completed his residency at William Beaumont Army Medical Center, El Paso, TX. Then he served as an OB-GYN with the Army at West Point, NY; Fort Huachuca, AZ; Fort Campbell, KY; Dhahran, Saudi Arabia; and Fort Bragg, NC.

He served proudly in the U.S. Army and retired after 24 years on August 1, 1995. Mark is a decorated Lieutenant Colonel, having been awarded the Army Commendation Medal, Army Achievement Medal, National Defense Service Medal, Southwest Asia Service Medal with Two Bronze Service Stars (BSS), Army Service Ribbon, Kuwait Liberation Medal, and the Expert Field Medical Badge.

He served others as an OB-GYN for over 45 years throughout the United States during and after his Army career. After discharge he spent the majority of his civilian practice at Logan Hospital in Russellville, KY, where his patients became part of his family. He also practiced in Nogales, AZ; Deming, NM; Utica, NY; and Evansville, IN. He and his wife, Janice, have spent over 16 years loving and caring for each other and have enjoyed vacationing together, and attending theater productions, with a special love for long cruises. He was a member of the Tucson Elks Lodge. He enjoyed spending time with family, playing games and cards, and watching sports. He was known for his great laugh and infectious smile.

A special thank you goes out to family, friends, and the agencies that helped care for him during his illness. A thank you as well to Drs. Kachaamy and Shrestha of the City of Hope Medical Center in Goodyear, AZ, along with his PCP, Dr. Gary Bohay, Tucson, AZ. Without the fantastic care and attention of this multitude of people, it would not have been possible to keep Mark in his home during his illness.

A Celebration of Life will be held to honor him in the summer in Wisconsin.

He is survived by

° His devoted, caring, and loving wife, Janice Lange Siletchnik (nee Brazeau);

° One son, Dustin Siletchnik of KY, Five grandchildren: Alaina (2023), Lauren (2016), Ari (2012), Alexander (2010), and Colin (2004);

° Two sisters, Gail Siletchnik of Middletown, RI and Karen Grimes (Mark) of DeLand, FL;

° Three sisters-in-law – Nancy LaPlant (Ken), Porterfield, WI; Joyce Gerdes Bitters (LeRoy), Tucson, AZ; and Carol Ghonsuli, Green Bay, WI;

° Three brothers-in-law – Steve Brazeau (Laura Burke), Oconto WI; Franklin Brazeau, Oconto WI; and Michael Brazeau, Green Bay, WI.

He will be missed by cousins, nieces/nephews, great-nieces/nephews, many friends, past patients, and others, as he touched many people. As we were recently reminded, "We all have our appointed time when Heaven Calls."

Friends and family are invited to attend his military ceremony on Thursday, November 14th, 2024, at 1pm, at which Mark will be honored with a military burial at the National Memorial Cemetery of Arizona, 2929 E. Pinnacle Peak Rd. Phoenix, AZ 85024. Please gather at the "Staging Area" by 12:45pm.

Guests are invited to the family home post-ceremony.
